New Nefrosoft® Renal Transplant module.
Within the program of continuous improvement of our products and services, the Visual Limes product development team has been working in recent months with professional nephrologists in the area of kidney transplantation, to incorporate new functionalities and improve this section of Nefrosoft®.
Once a transplant has been created, information can be recorded about: immediate postoperative period, hospitalizations, surgery and induction, complications (surgical, infectious, medium-long term), biopsies using the BANFF criteria, transplant immunologies, transplant consultation form, neoplasias, maintenance treatment, changes of immunosuppressant drugs and graft failure.
Nefrosoft manages the transplant waiting list with the patients of the application. Every modality of treatment has a common pre-transplant record where the data related to this functionality can be indicated, among the most important are:
-
First visit date transplant protocol
-
Situation on the waiting list (Under study, Included, Excluded temporarily or permanently, Temporary contraindication)
-
Date of inclusion in the list
-
Transplant doctor
-
Typing Center, including the code and reference number
-
Transplant hospital
-
Transplant registration number
-
Observations
-
Months on dialysis
-
Period on waiting list
A history of the changes on the situations in which the patient has passed is also kept. In the previous transplants card, all transplants can be indicated in the dates of onset, failure and the cause of transplant failure. With this information, the number of transplants of the patient can be extracted later.
Nefrosoft includes 3 types of transplant waiting list report with numerous filters per patient status, status in the list, etc. These reports can be edited by the functional group to adapt to the needs of the centers.
You can enter information from different type of donors like Alive or Corpse, if the donor is an Alive type, they can recover their demographic data from the external system, thus facilitating the user’s introduction.
Depending on the type of donor, some or other data may be recorded. The alive donor, as it must have a lifetime follow-up, has the whole common clinical history as the rest of patients.
The vast majority of selectable fields are configurable autonomously by the user.
An alive donor only allows the donation of one of the kidneys. Each kidney could be Accepted or Discarded for some reason or migrated, indicating the destination of the migration.
From each corpse donor you can register the clinical data, such as diagnosis, CRS, FG COCKROFT, etc. In addition, you can perform biopsy tests (SCORE) by returning a result. Depending on the numerical result, the system will show a detail configurable by the user in the ranges. The predetermined results are: Ideal for single transplantation, Acceptable for Double or Inadequate for transplantation.
When a transplant is created in Nefrosoft, the patient who receives it (Recipient) goes to the transplant treatment modality, where new files are enabled for the management of the transplant.
It is possible to record details of the transplant, such as type (Direct or simple) and sub-type (Chain, crossed, AB0 incompatible, etc.). Also data such as the weight in the transplant or the stay, calculated automatically with the income received from the HIS.
We can also introduce everything related to surgery, immunosuppressive pattern in the induction, immediate postoperative (can configure the debut of delayed renal function) and if you need dialysis, obtaining the number of sessions and the date of the last automatically.
The system allows to include several sensitizations of the transplant, having a data history that can be exploited at any time.
With reference to complications, all types of complications can be registered:
-
Surgical
-
Infectious
-
In the medium/long term
All the complications show the months since the transplant and depending on the type of etiology or the specific treatment is recorded. In infectious complications, the germ that causes it can be registered.
It is also possible to register Neoplasms, being able to exploit all this information in the form of consultations. We can enter the kidney biopsy information of transplantation, where some test questions are made based on the BANFF and can thus save the diagnoses related to the obtained result. These tests and diagnostics are configurable by the user, for editing autonomously.
A very important part of the transplant is the maintenance treatment (Immunosuppressant), where the drugs that belong to this group will be registered and all the changes produced and the reason for the change of one drug for another are stored. This information should be very detailed, because it is important for the doctor. In addition, it also allows the recording of adverse effects found in the patient related to a drug on a specific date.
Finally, in the transplant, Nefrosoft allows registering the failure of the graft, related to a change of modality, a patient’s loss or even an exitus. This screen will show the months since the transplant was performed.
